On the Olympic Programme for the second time, the mixed medley relay was won by Great Britain at Tokyo 2020, followed by China and Australia. On the World Championship programme since 2015, the United States had won the last three World titles.

Swimming a full B team, none of whom would swim in the final, the United States led the qualifiers with 3:40.98. Australia was second in round one, followed by China.

In the final, the USA elected to finish with women on the final two legs. Ryan Murphy gave them the lead on the opening backstroke leg, but China moved ahead after the breaststroke, based on a strong split by Qin Haiyang. China was also swimming women on the last two legs. Australia was back in fifth after the breaststroke, but had swum Kaylee McKeown on the opener.

Gretchen Walsh put the USA back in first after the butterfly, but barely so, only 0.04 seconds ahead of Zhang Yufei and China. Torri Huske was the American freestyler, and she brought home the gold medal, but it was always in doubt, as Yang Junxuan chased her to the wall, the United States winning by only 0.12 seconds in a world record 3:37.43. Australia, with Mollie O’Callaghan anchoring, took the bronze medal, more than a second behind China.
